The FoRT is an exam required for teacher licensure in Wisconsin (Early Childhood licensure, Elementary Education licensure, and Reading Specialist licensure). The test consists of 100 multiple-choice questions and two essay questions. A passing score in Wisconsin is 233.
The FoRT 190 replaced the FoRT 90. The topics covered connect to the FoRT 190:

This course is designed to increase learner’s success in school, work, and personal life by learning skills related to self-awareness, setting, and pursing meaningful goals, and developing positive personal qualities such as self-esteem, attitude, self-discipline, and self-motivation which will assist learner’s educational and vocation transition into independent living. This course students will start to develop their HESED Student Learning Plan (HSLP) and develop their personal and professional short term and long-term goals in collaboration with their advisor.

Faith Community/Parish Nursing and Congregational Health Ministries will focus on the Christian aspect of care giving in the church. The primary focus will be on the specialty of nursing called faith community nursing also known as parish nursing. Team taught by nursing and theology, the course is divided into 8 one week units which includes the theological foundation of Christian care giving, health and healing, and the meaning of pain and suffering. There is a strong emphasis on how to begin a congregational health ministry in a church and the nuts and bolts of faith community/parish nursing including roles and models of implementation. Faith Community Nursing is a nurse specialty that is recognized for the intentional care of the spirit, thus the student will be introduced to spiritual assessment and evidence based spiritual interventions. In applying knowledge learned, there is a culminating case study that focuses on the whole person including that of mind, body and spirit.
